I’m unsure of what to do

I had a breast reduction when I was 16 (I’m now 23) and about once a year a spot on my scar gets a wound (unsure as to how) and it gets infected it has happened again and I covered it with bandaid and used antibiotic cream and took it off and took a clean pit of tweezers and got all the gross dead skin mixed with puss off of it and am now letting it dry so now what do I do?
