If you've got it acccrued, use it! Thats what it's there for!!! I had heaps too and not much annual leave, so I took 4 weeks sick leave and with the way my roster and days off fall I'll actually have close to 5 weeks off which is awesome...
I've got a certificate from my PS saying I underwent a surgical procedure on this date and as a result am unfit for work till this date, thats all they should need!!
